(Exira) A car semi accident in Exira on Sunday afternoon claimed the life of an Exira woman.
The Iowa State Patrol says the accident happened at 2:48 p.m. at 418 Washington Street. Authorities say 75-year-old Phyllis Elaine Hoffman of Exira died in the crash.
According to the accident report, an eastbound Ford Focus driven by Phyllis Hoffman crossed the centerline on Washington Street and collided head-on with a westbound 2016 Mack semi-tractor and trailer driven by 24-year-old John Joseph Schultes of Exira. Hoffman died at the scene. Schultes was not hurt.
The accident remains under investigation.
