(Johnson Co.) One person was killed in a four-vehicle accident in Johnson County Sunday evening. Four others were injured.
The Iowa State Patrol says 41-year-old Thomas Richard Hebel Perkins, of Cedar Rapids, was driving a 2015 Jeep Cherokee westbound on 140th Street, approaching Highway 1, and failed to stop at the stop sign. The Jeep collided with the rear portion of a northbound camper being towed by 63-year-old Randy Ray Rouse of Mechanicsville. The Jeep then went through the rear portion of the camper and struck a southbound 2018 Toyota Rav4, driven by 77-year-old Usha Mallik of Iowa City. A 2011 Honda CRV, driven by 22-year-old Becky Lu of Bettendorf, struck flying debris from the camper.
Usha Mallik was pronounced deceased at the scene. Thomas Richard Hebel Perkins was transported to UIHC by Aircare. 34-year-old Samantha Paige Hebel Perkins of Ceder Rapids and a one-year-old child from Cedar Rapids were both transported to UIHC by Johnson County Ambulance. 46-year-old Matthew John Charles, of Paris, France, was transported to UIHC by Johnson County Ambulance.
The accident remains under investigation.
